Less effort per MD case.

Basen Health bundles records, drafts justifications and closes documentation gaps, so your medical controlling team can handle the rising audit load from 2027 without extra staff.

Case 2026-04418Fictional example case, cardiology
Audit subject: coding of principal diagnosis I50.13
Due in 12 days

Records

  • Discharge letter
  • Admission findings
  • Echocardiography report
  • Nursing documentation
  • Lab results

All requested records bundled

Justification

Draft, not yet approved

Based on the analysis, heart failure was chiefly responsible for the inpatient admission and is to be coded as the principal diagnosis (DKR D002). The admission findings describe dyspnoea on mild exertion Admission findings, and the echocardiogram shows reduced left-ventricular function Echo report.

Documentation gap: The NYHA class is not explicitly documented in the admission findings. For I50.13 it should be.

How does your hospital compare?

Compare your hospital with any competitor in Germany. It is based on the MD audit results of all around 1,600 hospitals in Germany.

Based on public data, calculated by Basen Health. No guarantee; subject to change. Customer data in the product: BSI C5-certified hosting in Germany..

From 2027, the share of undisputed audits sets the audit pressure.

Under the GKV-Beitragsstabilisierungsgesetz (GKV-BStabG), the MD audit rate depends on the share of audits that end without a reduction. Every reduction then costs three times: repayment, a surcharge and the lost expense allowance.

Audit rates from 1 January 2027

Set your hospital’s share
58 %

Only 672 of 1,525 rated hospitals in Germany reach at least 58 %.

The MD may auditup to 40 %of your claims.

74 % of hospitals with evaluable data in Germany are below 65 %, placing them in one of the two lowest tiers: up to 40 % audits, or no upper limit at all.

Sources: GKV-Spitzenverband, hospital-level data under § 275c (4) SGB V, Q1/2024 to Q1/2026; audit rates, surcharge and expense allowance under GKV-BStabG; own analysis.

What hospitals face from 2027.

The new rules land on teams that are already working at full capacity.

  • Far more audits

Below 65 % undisputed claims, the audit rate rises to up to 40 %. Below 50 %, there is no upper limit at all.

  • Every reduction costs three times

Repayment, a surcharge and the lost expense allowance: reductions become noticeably more expensive from 2027.

  • Same teams, more cases

Medical controlling and coding teams are stretched, and experienced coders are hard to hire.

  • Gaps start early

Whether an audit holds is often decided in the physician’s documentation, long before the MD case arrives.

  • Tight deadlines

MD cases come with fixed deadlines. Gathering records and drafting justifications takes most of the time.

  • A self-reinforcing cycle

    1. More reductions
    2. Fewer undisputed claims
    3. Higher audit rate in the next period

More audits bring more reductions, and the cycle starts again.

What Basen Health takes off your team’s plate.

Two building blocks that work together: the analysis of past reductions feeds directly into handling new MD cases. Case handling can start before the analysis is finished.

Building block 1

Case analysis and documentation quality

We analyse every MD case with a reduction and show where your documentation doesn’t hold up in an audit.

  • Reductions by department, DRG and audit reason
  • Documentation gaps, with examples from your own cases
  • A continuously updated guide for physicians

Effect: Fewer reductions through more complete documentation.

Works regardless of which software you use for MD management.

New in the physician guide: For heart failure, document the NYHA class explicitly in the admission findings.

Building block 2

MD case handling

For every new MD case, we prepare the records and the justification, based on the patterns from the case analysis.

  • Records for each MD case bundled automatically
  • Justifications drafted, with references to the file
  • Weak points spotted before submission

Effect: Less effort per MD case, stronger justifications.

We tailor case handling to the solution you have in place.

Embedded in your existing processes.

We adapt the solution to your workflows, systems and MD management, not the other way round. That way the partnership makes a difference in four places; we measure the actual impact together during the pilot.

50–70 %

less handling time per MD case: our pilot goal. Records are bundled automatically and justifications drafted.

  • Transparency

You know which departments, DRGs and audit reasons lead to reductions.

  • Better documentation

A continuously updated guide for physicians closes typical gaps and secures revenue in MD audits.

  • More team capacity

Medical controlling and coding handle significantly more MD cases with the existing team.

From first conversation to rollout, with results from the first meeting.

You receive a thorough analysis of your MD audit results in our very first conversation, before you commit to a pilot.

  1. Step 1: Analysis and alignment

We present the analysis of your MD audit results, benchmarked nationwide, and agree on goals and scope. NDA and DPA are signed.

Your result: Clarity on where your hospital stands.

  2. Step 2: On-site visit and data analysis

Over 1–2 days on site we document your coding, medical controlling and MD workflows and analyse your reductions.

Your result: An overview of where and why reductions occur.

  3. Step 3: Results and pilot decision

We present a report and a first physician guide. Together we decide on the pilot and set measurable goals.

Your result: Report, guide and a pilot with clear metrics.

  4. Step 4: Rollout

We embed the solution in your existing processes, support your team day to day and measure the impact against the agreed metrics.

Your result: Less effort per MD case, visible in daily work.

Privacy and systems

We work with your exports, not inside your systems.

Your HIS stays as it is. We only process pseudonymised exports, hosted in Germany and covered by a data processing agreement.

Data flow
  1. HIS and archivestay unchanged at your hospital
  2. Pseudonymisedtransferred under a DPA
  3. Basen HealthBSI C5 hosting in Germany
  • BSI C5-certified hosting

Your data is hosted and processed in Germany only, in a BSI C5-certified cloud environment.

  • Pseudonymised data

Data is shared in pseudonymised form. We sign a data processing agreement before any data is transferred.

  • Complements your systems

No change to your billing process: Basen Health works alongside your existing MD management software rather than replacing it.

  • Reading without an interface

We work with documents your hospital exports from its HIS and archive, for example as PDF or Word. No HIS interface is needed.

  • No changes to your HIS

We write nothing back to your HIS. Any later write-back would only happen through deterministic channels you approve, never through an AI agent operating the user interface.

  • Agreed with your IT

We agree data access, the operating model and technical requirements with your IT team at the start.

Frequently asked questions

What does the partnership cost?

Basen Health provides the project team and covers all development costs. After the pilot, partner hospitals receive preferred terms.

How much effort is it for our team?

A contact in medical controlling for about 2 hours a week, a 1–2 day look at your workflows on site, a data export from your existing systems and your feedback on our results.

Do we need an interface to our HIS?

No. We work with documents your hospital exports from its HIS and archive, for example as PDF or Word. We write nothing back to your HIS.

How is our data protected?

Data is shared in pseudonymised form and processed in Germany only, in a BSI C5-certified cloud environment. We sign a data processing agreement before the first transfer.

Does Basen Health replace our MD management software?

No. Basen Health complements your existing MD management software and does not interfere with your billing process.

When will we see first results?

In the very first conversation: we present an analysis of your MD audit results, benchmarked nationwide, before you commit to a pilot.

Where does the hospital check data come from?

From public hospital-level data the GKV-Spitzenverband publishes under § 275c (4) SGB V, analysed by Basen Health.
